2024 SUMMER CAMP INFORMATIONS AND DOCUMENTS

Hello Junior Wildcats! This is THE place for you to get ALL of the needed IMPORTANT information for you to leave for summer camp without surprises!!!

3. If you didn't read carefully the above PDF, we need you to COMPLETE this request BEFORE leaving. If you don't, you won't get paid for your summer camp!

Here are the instructions:

You need to fill and print this CDT-130 FORM, have a void cheque from YOUR PERSONAL BANK ACCOUNT.

It can't be your parents bank account.

For the SERVICE NUMBER BOX, you write nothing

For the UNIT BOX, in case you don't know it's 338

For the UIC BOX, you write 6929

By doing so, you will get money for having fun during summer camp!

4. Personal Property Log. To help the camps finding the owners of left behind things when you get back home.

Please have this filled out form with you at camp:

5. Personal Packing Checklist. Please print this and make sure you put X in ALL of the requested items:

6. Personal Code of Conduct: Please read and understand. (You don't need to print or sign) as long as you understand!

7. Medication Record: Filled, printed and with you at camp:

8. Time Off request form. If you need to leave camp:

9. Cadet Transportation Form You need to fill this If you take care of your transportation IN and OUT of the camps OR if your parents are picking you up after the final parade.
